The CIR process has been revised to enhance efficiency and account for the critical incidents that could negatively impact the health and welfare of young adults or their dependents, posing a risk of harm such as death, lifesaving medical emergencies, or unanticipated hospitalization. The categories listed in the attachment are considered critical incidents.
The CIR email notification will be sent to the required staff by noon on the first business day or within 24 hours. This notification will include a narrative containing information about the date and time when the service team was notified of the critical incident (CI), the date and time the CI occurred, the CIRT recommendation, the CI category, and a brief description of the incident that does not include any Private Health Information (PHI) or Personally Identifiable Information (PII). This information will also be copied and pasted into an activity log in SACWIS.
In lieu of using the CIR form, a more detailed narrative is noted in an Activity Living in SACWIS.
The Collaborative (Regional Coordinator and Administrative staff) and ODCY (Policy Developer) will review this information. Based on the documentation, we will determine if additional information is needed or if a CIRT is required.
The effective date is 1/1/25. We will review this new process during the December Regional Meetings.
